Process for Site Visits

To Apply for travel with Shoulder to Shoulder, please read and follow these steps:

1. Please read the Brigade Policies.
2. Fill out the Application.
3. View the Safety Video .
4. Complete the Safety Video Quiz .
5. Print, sign, scan and email the Waiver to Sharon.M@shouldertoshoulder.org
6. Physicians and Dentists, scan a COLOR copy of you license and send it to Sharon.
7. Send the appropriate fees to Sharon as listed in the Brigade Policies
8. Absolutely no one will be permitted to travel with Shoulder to Shoulder without completing all the steps above including viewing the video and submitting the quiz.
